HTML template for the print header. Should be valid HTML with the following classes used to inject values into them: - `date` formatted print date
|

- `title` document title
|
|
|
|
- `url` document location
|
|
|
|
- `pageNumber` current page number
|
|
|
|
- `totalPages` total pages in the document
|
|
|
|
